PARENTAL AGREEMENT: 

 

I (we) grant the Fox Valley Chinese School, hereinafter called FVCS, the power to authorize medical treatment or medical procedures, at my (our) sole cost and expense, in an emergency situation to aid me (us) and/or my (our) child(ren) in connection with summer camp organized by FVCS.  In further consideration of the services and instruction provided, I (we) hereby release FVCS, its officers, teachers and volunteers from any and all claims or liabilities which may result from participation in the summer camp, including but not limited to those arising as a result of negligence.  As a condition of enrolling my (our) child(ren) in any activities at the camp organized and/or sponsored by FVCS, I (we) agree to adhere to and accept all the summer camp policies and rules.
